Welcome to Empowering Clients as Leaders of Their Care: Creating Advanced Directives and Avoiding Guardianship. This program has been designed for Continuing Education for Social Workers and other Health and Social Service professionals. The program is offered within the Apples of Gold projects collaboratively developed by Access Care Services, LLC and MAWMedia Group, LLC. The owners each have more than 30 years of practice in the health and social service industries, as well as higher education. Both are committed to empowering people to be the leaders of their care, as well as strengthening Social Workers and other helping professionals to continually improve the quality and outcomes of our services to the clients and families we can serve as our world and the challenges we face change. The purpose of this training is to prepare social workers and other health care professionals with the knowledge, skills, and confidence necessary to educate and assist all decisional adults, from HS seniors to Senior citizens with advanced planning. This includes assisting clients to understand the need for advance planning and options for completing Health Care Advance Directives, as well as providing guidance to complete and maintain Health Care Advance Directives. Helping clients understand and document their needs and wishes is one step toward empowering people to be leaders in their care.
